The Health Clarity Planner is a printable reflection resource for organizing everyday observations and records. It is not a diagnostic tool, medical screening test or personalized treatment plan. You can use it without a wearable and without collecting a perfect set of measurements.

Use a typical week
Record your sleep opportunity, energy, ordinary activity and the circumstances you think matter. Keep notes short enough to sustain. Distinguish what you felt from what a device estimated, and keep original test reports rather than copying isolated numbers without their units.

Prepare a question from the notes
Choose one pattern you would like explained, state when it began and describe how it affects daily life. Bring relevant medicines and original records to an appropriate professional. Do not interpret a completed planner as proof that a symptom is harmless or that you need a particular supplement.

When a planner is not the next step
Persistent or disruptive symptoms need appropriate assessment. Urgent symptoms need local care, not another week of tracking. If monitoring increases anxiety, reduce the detail and discuss the concern; the value is a clearer conversation, not achieving a flawless diary.
