Distinguish what you felt from what a device estimated or a laboratory measured. Include when the issue began, how it changed, relevant medicines and what another provider has already advised. Original records reduce the risk of important detail being lost in a summary.

A useful answer explains what is known, what remains uncertain and what the next step is. Ask why that step is appropriate, what alternatives exist and what changes should prompt further care. Do not alter prescribed treatment solely because of a website or AI-generated explanation.
