Before treatment begins, ask which outcomes and safety observations matter and how often they are reviewed. Progress may include everyday function, tolerability and health goals as well as weight. A plan should identify what to record and which changes need professional attention.

Keep a short record that supports the review
Note the treatment actually used, relevant dates, symptoms and practical difficulties. Include wearable trends only when useful and label them as estimates. Do not collect more sensitive data than the care team needs, and use the provider’s secure channel to share personal information.

Know who to contact between appointments
Clarify how to report unwanted effects, missed doses or an interrupted supply and when a response can be expected. Follow medicine-specific instructions and the clinician’s plan. Emergency concerns should go to local emergency services rather than a routine digital follow-up.

Review the plan when circumstances change
Tell the clinician about new medicines, conditions or major changes affecting the agreed approach. Ask whether monitoring or the plan needs to change. Do not alter a prescription simply because a wearable score improved or worsened; the provider must evaluate treatment decisions.
